If you are a patient, you will enroll via an email link once your practitioner certifies you.

• Health Care Practitioner – Create Account
  Register as a health care practitioner in the Minnesota Medical Cannabis Registry. You must be a Minnesota licensed Physician, Physician Assistant or Advance Practice Registered Nurse to register as a health care practitioner in the Minnesota Medical Cannabis Registry. To register, you will need:
  • General information: name, address, email and phone.
  • Your medical license.
  • Your DEA license.
  • General information about the clinic(s) where you practice.

It may be a crime under MN Law to impersonate a health care practitioner.
