Maryland Medical Marijuana Card Application

 

  1. Get a copy of your medical records from your primary care physician describing your medical diagnosis
  2. Get a signed Physician’s Statement from a licensed Maryland physician.
  3. You must be at least 18 and a Maryland resident.
  4. You must be diagnosed by a physician with a qualifying medical condition:
    • A chronic or debilitating disease or medical condition that results in a patient being admitted into hospice or receiving palliative care; or
    • A chronic or debilitating disease or medical condition or the treatment of a chronic or debilitating disease or medical condition that producesCachexiaanorexiawasting syndrome, severe or chronic pain, severe nausea, seizuresor persistent muscle spasms
  5. You must register with the Maryland Natalie M. LaPrade Medical Marijuana Commission.
  6. You must pay the patient registry fee.

Maryland Medical Marijuana Physicians Registry

The certifying physician registration form as required by COMAR 10.62.03 is now available. If you are interested in becoming a certifying physician, please  click onto this link: mmcc.dhmh.maryland.gov to complete the on-line registration form. You will need your Maryland physician’s  license number and controlled dangerous substance (CDS) number to complete the form.

After successfully registering, you will receive an instant e mail message indicating that your registration has been processed. Please note that this message will make reference to the patient registry and the written certification form. At this time, these documents are not available on the website, but will be forthcoming in early 2016.
